Help Needed Still starting troubles

, Our truck still has starting troubles, new glow plugs, relay and now a IDM, from the stupid dealer who said it was the problem and it wasn't. Has to be plugged in no matter what the temp outside, when it starts it idles really rough for about a minute, dies then on a re-start idles fine. Now it also has hicup? stutter, shaky thing at around 55k, 75k, and when you give a good bit of fuel it runs totally smooth until you let off the gas pedal, doesn't always do it either, only sometimes. Sure could use some help on the most likely place to start. It was at the dealer and they said for sure the IDM, after inspecting it with the valve covers off, could the CPS cause some of this as it hasn't been replaced.
or wiring harness????

what color is the smoke? if any?

it isn't smoking, was running a some black smoke back in the summer, some injector cleaner seemed to take care of that, I will add this shaky thing, rough idle, whatever it is started after we had a diesel shortage here, were getting the dregs from the tanks, they were actually running out of fuel at most gas stations. Had a bit of grey smoke, but not now. thanks

Did the dealer check the injection control pressure? Have you run additive though it again?
