I think the trick is to have 2 lady's slippers...one on either side so that you can pry up evenly. If it has never been out, it's probably crudded up pretty good and won't want to come out easily.
On really, really cold mornings...not that it gets all that frigid in Jersey, I'll simply cycle the glow plugs. It's waaaaaaay cheaper than pluggin' in the block heater. I've never had a hard start.
Interestingly, my truck does the same thing...but only when I'm in stock programming mode. When I load my street or tow tune, no smoke at take-off. Go figure.

What lift height did the Xs give you? I've heard various numbers...not one has been the same. The number wasn't verified by someone who actually had Xs installed, however.
