I replaced my manual factory hubs with a set of WARN Premium manual hubs, part # 38826. Very easy to install.

The 4x4 duallys just run a spacer bolted onto the rotor. The 2wd duallys have a a one piece brake rotor like the older GM duallys do. Could the hole inside the spacer be too narrow?
