Jerking Issues

I'm still having problems and not getting anywhere, my truck sometime jerks hard and the RPM's jump, other times it just stall's out, it throws the check engine light and also the SES light. I just replaced the fuel filter(No Water in fuel), the CPS, and the ICP sensor. Could the P1249 code or the P0470 code have anything to due with these issues. Code below are what keep coming up.

I took my diablo chip out, truck doesn't stall or jerk anymore (3 days). Not throwing any codes either. Hopefully it was just chip related.
